Take the Test

1. This competition is open to residents of the Republic of Ireland aged 18 and older.
2. Laya healthcare employees, their agents, families or anyone professionally connected with the promotion are excluded from the competition.
3. To be included in the competition draw entrants must answer the five questions correctly and submit their contact details.
4. The competition will run from Monday 6th April, 2015 at 9am and will end at 12pm on Sunday 12th April, 2015. A winner will be drawn on Monday 13th April.
5. Winners will be contacted via email.
6. The winner’s name will be announced on the laya healthcare Twitter and Facebook social media channels.
7. The prize consists of private health insurance for a family of 2 adults and 2 children for one year based on our Assure First scheme, which is the lowest cost hospital scheme on the market.
8. The prize is to the maximum value of €990 based on the price as of 13th April 2015.
9. Prizes are not transferable for any other alternative.
10. Only one entry per person will be entered into draw
11. Laya healthcare’s decision of winner is final and no correspondence will be entered into.
12. In the event that laya healthcare is unable to contact the winner within 30 days, it will draw a replacement winner and the first winner will no longer be eligible to receive the prize.
13. Laya healthcare reserves the right to suspend or terminate the competition, extend the closing date or amend these terms and conditions.
14. By entering the competition you agree to accept these terms and conditions.
